Our Healthy Aging Program is delivered by Angela Draper RN and Daniella Fry OT.
The Healthy Aging Program has been developed for older adults faced with challenges of
aging. For example you may have problems with falls, balance, osteoporosis, arthritis, cognition and other individual health concerns identified by your family doctor.
The goal of the program is to work with you to identify attainable and realistic self-care
management strategies to maintain and improve your health to the best of your ability.
The Registered Nurse and Occupational Therapist will provide patient centered assessment and health teaching and help you link to appropriate exercise programs and community resources. Our team may also refer you for Geriatrician assessment via the telemedicine network (OTN).
With the patient/family, Angela and Daniella will develop a plan of care that will be communicated to your doctor.
The Healthy Aging Program can help link patients to programs such as Home for Life, First Link Alzheimer’s, 211, Pharmacy Meds Check, Integrated Regional Falls Program, Nordic Pole Walking, GBFHT Balance Program (exercise for balance, strength, mobility, falls prevention), VON Smart, Arthritis Society Physiotherapist, and other appropriate services.
