What is a Family Health Team?

Family Health Teams are primary health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide primary health care for their community.

To access our services, you must be a patient of a Family Health Team doctor and have a referral from your doctor or another Family Health Team care provider.

Mental Health Clinical Access Coordinator

Location: Collingwood, ON

Start date: May 2022

FTE: Permanent full-time

Hours of work: Predominantly 8.30am-4.30pm Mon to Fri, one day/week 11.00–7.00pm.

We are seeking an experienced Registered Social Worker, Registered Psychotherapist, Registered Occupational Therapist, or a Registered Mental Health Nurse to join our mental health team. Our region offers the very best of summer and winter activity for those with a passion for the outdoors. Our progressive, multi-disciplinary team is a key player within the South Georgian Bay region, proud of the solid relationships and history of collaboration with all our community partners. We work in a professional and collegial environment, use an up-to-date electronic medical records system (EMR), and offer many opportunities for ongoing training and professional development. The Georgian Bay Family Health Team (GBFHT) provides a host of primary care services including Mental Health, Lung Health, Diabetes, Healthy Aging, Nutrition Counselling, Cardiovascular Rehabilitation and Ontario Telemedicine Network (OTN).

Our mental health team, including Child, Youth, and Adult counsellors, provides exceptional primary mental health care service to patients of the 52 area physicians who comprise our Family Health Organization.

The successful applicant will work collaboratively with the Access Coordinator/Team Lead to provide intake, triage, stabilization, care coordination, systems guidance, and transition support for GBFHT MH patients. The primary objective of this role is to provide a seamless approach to primary mental health care and to facilitate efficient, effective, and appropriately paced system navigation support. This role will be balanced with individual support, groups, and patient/family mental health education.

Key Responsibilities of this Position:

  • Competent telephone/virtual and in-person mental health screening, triage and needs assessment.
  • Competent provision of brief phase-oriented counseling within a stepped care framework
  • Determine treatment approach, clinical environment, and therapeutic modality for patients/families with particular attention to the needs of vulnerable populations.
  • Connect patient/family to resources and services in a sensitive, supportive, and participatory way that ensures access and minimizes barriers.
  • Thorough understanding and knowledge of community/regional/provincial mental health resources, referral processes and criteria, as well as peer and self- support alternatives.
  • Facilitate and support the establishment of barrier free and efficient referral pathways in collaboration with other community professionals/services.
  • Provide mental health system support and guidance to the multi-disciplinary team, Primary Care Providers, and Specialists
  • Represent GBFHT at collaborative/community tables and regional/local mental health service design, development, and evaluation projects.
  • Develop resource partnerships with other community members/partners.
  • Provide mentorship, supervision, and evaluation of students in professional training programs.
  • Maintain records in accordance with professional college documentation standards and guidance.
  • Manage a full and demanding workload, with competing priorities.

Education and skills:

  • Master’s degree in counselling, occupational therapy, nursing, social work, or psychology.
  • Current registration and a member in good standing with the Ontario College of Social Workers, College of Registered Psychotherapists of Ontario, College of Nurses of Ontario, Ontario College of Occupational Therapists, or College of Psychologists of Ontario.
  • Minimum of 5 years mental health, counselling and/or psychotherapy experience.
  • Capacity to provide support across the lifespan including children, youth, parents, young adults, families, couples, and older adults.
  • Certification and/or training in a broad range of therapies with emphasis on brief therapy, CBT, IPT, DBT, EFT, MBSR, and CPT among other evidence-based approaches.
  • Proficient suicide risk assessment, conflict management, and de-escalation skills.
  • Skilled interdisciplinary team member that values professional development, interprofessional feedback, and active participation in team supervision processes.
  • Excellent interpersonal, communication and networking skills required.
  • Excellent organizational and time management skills required.
  • Excellent reasoning, analytical and problem-solving skills required.
  • Familiarity with EMR PS Suite an asset.
