About Us

MISSION STATEMENT

We are a collaborative team of interdisciplinary health care professionals delivering quality health care in a patient-centered environment.

What is a Family Health Team?

Family Health Teams are primary health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide primary health care for their community.

Am I Eligible?

To access our services, you must be a patient of a Family Health Team doctor and have a referral from your doctor or another Family Health Team care provider.

Find out if your doctor is part of our team.

Exceptions:

If you have Diabetes and do not have a doctor that is part of our Family Health Team, please contact us.

“Just In Time” Counselling is open to everyone.

Job Opportunities

Registered Nurse (RN) Chronic Disease Management

Location: Collingwood, ON

Start date: November 2021

Job Type: Full time permanent

We are seeking a Registered Nurse, with a passion for patient education and chronic disease management.

Our region offers the very best of summer and winter activity for those with a passion for the outdoors. Our progressive, multi-disciplinary team is a key player within the South Georgian Bay region, proud of the solid relationships and history of collaboration with all our community partners. We work in a professional and collegial environment, use an up-to-date electronic medical records system (EMR), and offer many opportunities for ongoing training and professional development. Together we ensure that patients of the 52 area physicians receive optimum care.

Your role would support a diagnosis of hypertension in setting up and reporting on data collected through Ambulatory Blood Pressure Monitoring. You will deliver education and advice to enable patients to better manage their hypertension. You will work with our Certified Respiratory Educator (with opportunity to work towards this designation) in our smoking cessation and pulmonary rehabilitation program. You will work alongside our Registered Kinesiologist in delivering the cardiovascular rehabilitation program in partnership with the YMCA Simcoe Muskoka. You will also be called upon to support clinics through our telemedicine program.

This role will require a great deal of versatility, critical thinking and the ability to problem solve quickly. Training will be provided in the use of OTN equipment used to inform the clinical assessment.

 

Necessary skills:

  • Strong technical and organizational abilities.
  • Exceptional communication skills both written and verbal are essential.
  • Ability to multi-task.
  • Demonstrated commitment to increasing knowledge and practical application of the Canadian Hypertension Education Program guidelines and the Canadian Respiratory guidelines.
  • Demonstrated commitment to self-directed learning.
  • A passion for chronic disease management and enabling people to achieve their goals

 

Qualifications:

  • Registered Nurse in good standing with College of Nurses of Ontario (CNO)

 

Proficiencies:

  • Experience in community and/or primary care setting preferred
  • Experience in the area of chronic disease management and prevention preferred
  • Demonstrated skills in facilitating groups preferred
  • A positive outlook and a willingness to “pitch in”
  • Strong computer literacy skills essential
